Job Summary
Under minimal supervision, performs in-patient nutrition assessment, re-assessment and education. Implements Medical Nutrition Therapy (MNT) to optimize patient care and outcomes. As a member of the multidisciplinary healthcare team, actively participates in patient care rounds. Assists Registered Dietitian Supervisor with PI/CQI activities to ensure quality patient care.
Under minimal supervision, performs nutrition initial assessment, re-assessment and education for bariatric patients.Implements Medical Nutrition Therapy (MNT) to optimize patient care and outcomes. Performs inpatient and outpatient visits for bariatric patients in preparation for, and following bariatric surgery. Monitors patient’s nutritional status and needs in preparation for, and following bariatric surgery. Maintains an outpatient clinic, including scheduling patients, performing visits, providing and reviewing educational materials, arranging follow up, monitoring patient progress through the program, and recommending readiness for surgery. Follows patients in house immediately after bariatric surgery, and follows patients in outpatient visits after surgery to ensure optimal nutritional status and understanding of nutritional and behavioral goals. Maintains up to date bariatric nutritional educational materials. As a member of the multidisciplinary healthcare team, actively participates in patient care rounds and team meetings.Assists the Bariatric Medical Director, Bariatric Coordinator, Bariatric APPs, and Bariatric SCR with PI/CQI activities to ensure quality patient care. Leads, prepares content and educational materials for, and participates in bariatric seminars and support groups.
Responsibilities
1.Prioritizes work load appropriately.
2.Provides education with respect to the patient/family’s education level, optimal learning tool(s)/techniques(s) and readiness to learn.
3.Provides appropriate education and documentation.
4.Provides patient education according to policy.
5.Regularly participates in physician rounds, discharge planning and team conferences as a multidisciplinary team member to ensure optimal patient care.
6.Participates in quality improvement and/or patient care committee/task force as assigned by supervisor.
7.Completes monthly productivity logs.
8.Performs PI/CQI projects as assigned by supervisor.
9.Routinely identifies areas makes suggestions to improve service and care.
10.Effectively obtains patient information and handles patient/customer requests and complaints
11.Documents patient care activities on appropriate forms and according to accepted hospital practice.
12.Performs intake analysis (calorie counts) as needed.
13.Accurately clinical activity data into computer tracking program.
14.Other duties as assigned.
Knowledge, Skills & Abilities
Minimum Qualifications Education and Experience * Bachelor’s degree in nutrition, dietetics or directly related field. Masters degree preferred. * Typically, one to three years related experience in clinical or public health nutrition. Knowledge, Skills and Abilities * Effective communication skills in both oral and written form. * Demonstrates ability to delegate responsibility as appropriate. * Ability to exercise good independent judgment. * Ability to follow written and oral instructions. Process the ability to assess data reflecting the patient’s status and interpreting appropriate information. * Demonstrates knowledge of disease specific and age specific nutrition standards of care. * Demonstrates understanding of Local, State, Federal and The Joint Commission standards and regulations. * Demonstrates competency with computer system for order entry and to update diet and patient information. * Demonstrates understanding of how to use most current version of on-line Nutrition Care Manual. Computers and Technology * Ability to use Microsoft Word, Outlook, Excel and other IT/software systems. Certificates, Licenses and Registration * Registered Dietitian. Maintain current registration credentials and requirements of the Commission on Dietetic Registration. All Certifications and Licenses required for this job must be kept current as a condition of employment. Supervisory Responsibilities This position has no supervisory responsibilities
Education and Experience
1.Bachelor’s degree in nutrition, dietetics or directly related field required.
2.Master’s degree in same field preferred.
3.Two years of related experience in clinical or public health nutrition required.
