DESC is a multi-service agency that specializes in providing permanent supportive housing, emergency shelter, residential crisis services, and comprehensive behavioral health outpatient services for people with long-time experiences of homelessness and serious behavioral health disabilities. We utilize Harm Reduction, Trauma Informed, and Housing First practices, striving to deliver flexible services that accommodate consumer’s unique needs.
JOB DEFINITION
In conjunction with the Associate Director of Outpatient Behavioral Health, the SAGE Program Manager oversees all outpatient behavioral health (OPBH) treatment activities related to mental health, including integrating with medical and substance use teams to form long-term, whole person recovery and case management services. This position leads teams sited at (3) outpatient behavioral health clinics, DESC permanent and supportive housing locations, and scattered sites. Outpatient behavioral health works closely with other DESC programs to assure successful continuity of care and promotion of recovery to the highest degree possible.
The SAGE PM must be willing and able to work in a fast-paced environment where staff availability and program needs are often changing. This position will work closely with (5) Sr. Housing/Shelter Program Managers, and (9) Sr. Clinical Program Managers toward providing seamless services within the agency, as well as the community. They will also work in coordination with the Clinical Programs Coordinator and Quality Information Management (QIM) team related to Medicaid Integrated Managed Care (IMC), Collective Medical Technology online platform, Washington Administrative Codes (WAC), Washington State laws (RCWs), King County Integrated Care Network Provider Manual, and HIPAA privacy rules. They will have primary responsibilities for assuring compliance with all local, state and federal laws and relevant contract and procedure requirements associated with the operations within the SAGE programs. Time will be spent coordinating with other support departments within DESC to help the coordination of care and integration of services wherever DESC OPBH services are provided.
MAJOR DUTIES AND RESPONSIBILITIES
Supervision and Training
  • Provide administrative and task supervision for (2) SAGE Project Managers and co-supervise (1) SAGE program coordinator with Clinical Programs Coordinator.
  • In conjunction with the Project Managers, helps provide oversight to (6) SAGE program supervisors,
  • Coordinate, hire, and manage clinical supervision contractor for up to (6) SAGE program supervisors.
  • Attend scheduled and routine team meetings with program supervisor and direct service staff.
  • Conduct performance evaluations of direct reports on scheduled basis and maintain supporting documentation in confidential files.
  • Educate staff regarding agency policies and procedures and monitor for compliance.
  • Maintain a system for training staff in all core competencies.
  • Supervise and review clinical documentation in order to assure that clinical staff maintain current and complete clinical records, and that records comply with the agency's clinical accountability policies and procedures.
  • Teach and promote outreach services and active integration of recovery-based service with all clients.
  • Plan and arrange staff training as needed.
Contract Compliance & Quality Assurance
  • Maintain familiarity with relevant licensing, contract, regulatory and procedural requirements associated with delivery of all SAGE services and maintain service standards.
  • Write and refine procedures as needed.
  • Plan and coordinate program audits and site visits by funding and regulatory bodies.
  • As required, develop and implement progressive discipline and corrective action plans.
  • Maintain a high program accountability through chart review and database reports to assure timeliness and accuracy of program work.
  • Participate as a regular member of standing agency and ad hoc committees as assigned by clinical director.
  • Complete program reports as needed.
  • Maintain strong knowledge of DESC personnel policies, procedures and collective bargaining agreement. Assure compliance with same.
  • Resolve client complaints and grievances.
  • As directed, represent DESC at external meetings.
  • Assure implementation of program development in congruence with principles of recovery-based services.
  • Assure strong promotion of paid employment, internships, and volunteer work as an outcome for clients.
Program Management
  • Coordinate with DESC programs to assure successful transition and coordination of care for consumers to SAGE services from other clinical programs, PSH, and shelter programs.
  • Collaborate and problem solve with other DESC program and departmental staff to ensure tight continuity of care for consumers.
  • In conjunction with other DESC and Harborview Medical Center Leadership, provides operational support to Hobson Clinic.
  • Coordinate and participate in psychiatric consultations, clinical supervisions, program meetings, and in-service training of mental health program staff; participate in clinical reviews and case conferences.
  • Schedule and conduct quarterly quality assurance reviews of clinical charts.
  • Oversee the implementation of the HealthierHere Care Coordination Innovation Project with Harborview Medical Center.
  • Oversee compliance with documentation requirements of relevant WACs, KCICN Medicaid model, PHP and other funding sources (as applicable), in conjunction with Data Systems Administrator and staff.
  • Establish and maintain cooperative relationships with human service organizations that provide resources needed by mental health consumers and with other agencies in the mental health and social service system.
  • Assure adequate staffing at all SAGE service sites.
  • Assist with development and review of budgets as directed.
Service Coordination Internally & Externally
  • Participation in Scattered Sights Master Leasing, Seattle Housing Authority Clinical Vouchers, Greenwood House; Shelter Plus Care (SPC)-SRA and Shelter Plus Care (TRA) contract renewal, budget oversight and audits.
  • Monthly meetings with the Scattered Sites Manager to review SPC outreach metrics, match, problem tenants, and paperwork requirements.
  • Close collaboration with the Housing SPM who oversees housing contracts that are served by SAGE.
  • SPC Housing Specialist join SAGE team meetings and/or SAGE Supervisor meetings monthly depending on the need.
  • Meet monthly with the other Clinical, Permanent Supportive Housing, and Shelter Managers for coordinating continuity of care and service delivery-site/program specific.
  • Establish and maintain positive relationships with relevant community members and organizations.
Recruitment and Hiring
  • Work in coordination with Human Resources to establish and maintain a recruitment plan aimed at hiring and maintaining a diverse and competent staff that is reflective of the client population DESC serves.
  • Coordinate with Human Resources staff to advertise and conduct hiring in a consistent, effective and cost-efficient manner.
  • Maintain familiarity with agency and mental health system policies and procedures and assure adherence to these procedures. Identify need and assist in development and modification to procedures.
  • Other duties as assigned.
MINIMUM QUALIFICATIONS
  • Meet minimum criteria for MHP as defined in current RCW & WAC.
  • Master's degree in social work, psychology or other relevant behavioral science or Bachelor of Nursing degree with specialty in mental health.
  • Minimum of 2 years full time experience delivering mental health and case management services to severely mentally ill adult population in a community setting.
  • Two years' experience supervising clinical staff.
  • Possess the skills and attitude to provide sound clinical and administrative supervision.
  • Strong written and verbal communication, problem-solving, analytical and leadership skills
  • A personal vehicle is not a requirement of this position. However, if using a personal or agency-owned vehicle in the performance of job duties associated with this position, employee must possess and maintain a valid Washington State Driver’s License, the appropriate amount of automobile insurance, and a safe driving record.
  • Ability to work flexible hours as required by program and staffing needs including occasional evenings or weekends.

PREFERRED QUALIFICATIONS
  • Qualified to provide approved supervision to LICSW and LMHC candidates.
  • Understand and can demonstrate how the DESC core values are incorporated into the day-to-day clinical work.
  • Have experience working as a supervisor or manager in an integrated environment, experience managing milieu.
  • Experience in crisis management
