2. Accompany each resident on all medical visits. Stay with resident(s) until visit is complete.
3. Observe the following guidelines at each visit:
a. Sign in resident upon entrance.
b. Present resident’s documents to office personnel.
4. Before leaving doctor’s office, please observe the following:
a. Obtain resident’s documents.
b. Obtain any new prescriptions.
c. Check to make sure doctor has written on the doctor’s order form.
d. If follow-up appointment necessary, obtain appointment cards.
5. After returning to facility, report to RCM and be prepared to answer questions concerning visit.
6. Performs other duties as required and assigned.
- Physically able to help ambulatory and non-ambulatory residents transfer to and from wheelchair to vehicle.
- Possess a valid state driver’s license.
- Qualified as a Certified Nurse Assistant (C.N.A.).
- Clean driving record.
- Clean criminal record.
- Any other state required certifications
