JOB OBJECTIVES:
To support individuals with a diagnosis of development disability and/or related conditions. This position provides individualized assessment, education, and service coordination, working collaboratively with individuals, families, provider agencies and community partners to ensure each individual has the supports and services needed for a healthy, safe, and fulfilling life. This role will require understanding the various services offered by Douglas County Social Services (DD case management, CMH case management, CW case management), assessment of the youth and family’s needs, and making a recommendation based on the assessment process regarding best fit.ESSENTIAL DUTIES AND RESPONSIBILITIES: The duties listed are intended only as illustrations of the various types of work that may be performed. Education
High School diploma or G.E.D.
A Bachelor's degree from an accredited four-year college/university with a major in social work, psychology, sociology, or a closely related field.
A Bachelor's degree from an accredited four-year college or university with a major in any field and one (1) year of experience as a social worker in a public or private social services agency.

- Assist applicants seeking case management through assessment and coordination of services to determine program eligibility, explore healthcare options, understand process and timelines, and make recommendations or referrals to appropriate resources.
- Provide person-centered assessment and case management services to locate and gain access to needed services.
- Provide person-centered assessment of children who may be dual eligible for Developmental Disabilities and Children’s Mental Health (DD/CMH) and potential case management.
- With the individual’s/family’s involvement, develop and implement a person-centered case plan that is based on the assessed and identified needs.
- Evaluate, facilitate, and monitor services and individual progress in relationship to established goals and objectives. Works with the person to adjust service plans as necessary.
- Plan and arrange for services and facilitate appropriate payment.
- Participate in review of individual’s Consumer Directed Community Support and Family Support Grant plans.
- Become certified in and conduct MnCHOICES assessments as needed.
- Act as a liaison with community and referral services.
- Prepare and maintain case records which meet federal, state, and local statutes/rules.
- Under direction of the supervisor, seek court intervention when needed to protect the health/safety of vulnerable children or adults: prepare reports needed for court, testify in court as may be necessary, and implement court orders.
- Participate in team meetings, agency staff meetings, county meetings and trainings.
- Perform other duties as assigned.
