What are the responsibilities and job description for the LPN position at Eastway Behavioral Healthcare?
Responsible for the provision of professional nursing care to clients of Eastway Behavioral Health. Coordinates medication/somatic services that address client needs. Provides information regarding client conditions and responses to medication according to physician orders. Follows physicians' orders and documents client care requirements and process in the clinical record.
Duties and Responsibilities
- Coordinates medication support services with community support program personnel in order to insure provision of effective, efficient, and safe client care in accordance with established state and agency standards.
- Assesses and evaluates client needs as related to service planning efforts to assure that mental health issues are addressed.
- Consults with community support program Staff in the formation, implementation and delivery of individualized service plans. Assists other behavioral medicine staff in coordinating client care,
- Performs nursing assessments on referred clients.
- Compiles and communicates information to the community support program staff and/or behavioral medicine staff on continuity of client care issues.
- Provides nursing care to clients including, but not limited to, the administration of medication; implementation of nursing procedures as ordered by the physician or as indicated by the nursing assessment; verbal and physical interventions in accordance with professional standards.
- Initiates crisis alerts when appropriate for clients who present with exacerbation of symptom’s which require crisis management plans.
- Conducts health assessment consultations when problematic health histories are reported by clients.
- Provides written/oral information on medications in a language and fonnat that is understandable to the person receiving it.
- 10.Performs other similar or related duties as required or assigned within this job class.
Must have LPN License or RN License
Hours: Monday through Friday 8-5pm
Pay: $23 to25.00 based on experience
