Aurora on France TCU is seeking a licensed Social Worker, LSW whom would help Transitonal Care Unit adult Short Stay patients and their family understand and cope with the psychosocial aspects of their lives involving change, grief, and loss through crisis intervention - individual, group and family counseling. Located across the street from Fairview Southdale Hospital in Edina, MN.
- Full Time Hours; 80 Hours Per Two Week Pay Period
- Day shift
- No weekends

- Coordinates admissions with the admissions team as appropriate
- Manages resident caseload with significant turnover to accept/discharge new patients
- Provides one to one and group counseling as appropriate
- Serves as a Discharge Planner
- Assists in Vulnerable Adult investigations and completes necessary documentation and reports.
Bachelor's Degree from an accredited school of social work
Licensed by the MN State Board of Social Work
One to three years experience in LTC
