The Nurse Liaison, RN is responsible for enhancing and managing Hennepin County Medical Center (HCMC) hospital-based referral relationships through formal and informal network development, representing Ecumen Hospice as the provider of choice. This position will leverage opportunities to bind referral source loyalty by creating a positive experience between the referral provider and Ecumen Hospice. This position is also responsible for identifying opportunities within HCMC that develop successful relationships with quality and value for the patient.

Essential Job Responsibilities:
- Works closely with hospice leadership, hospice intake and hospital staff of assigned clinical service areas to facilitate hospice referrals and admissions.
- Gains internal hospital support for appropriate hospice referrals and admissions by attending various rounding opportunities and responding to referral requests.
- Prepares reports to identify trends, opportunities, and barriers to hospice referrals and admissions to share with hospice and hospital leaders.
- Coordinates referrals and inquiries to build and maintain relationships and offers hospice education.
- Collaborates with hospice intake, the patient, and hospital staff to ensure a smooth transition to hospice care.
- Collaborates with theHospice Care Consultant regarding other HCMC referral opportunities (e.g., clinics) that are not on the hospital campus and/or the need for collateral for internal hospital use.
- Identifies service gaps and brings them forward to the hospice leaders for discussion and resolution.
- Current license to practice as a registered nurse in the State of Minnesota
- 3-5 years of clinical experience
- Professional and effective oral and written communication skills
- Basic computer skills
- Strong clinical and relational skills
- Ability to analyze situation and implement solutions
- Ability to handle multiple priorities, ambiguity, frequent change and high stress situations.
- Bachelor's of Science in Nursing
- 5 years of Hospice experience
