Position Summary
Position exists to lead Intensive Outpatient and SUD Outpatient groups, complete intakes and provide case management and case coordination for persons suffering from addiction. May provide mental health and/or substance abuse counseling for clients who suffer from mental illness, developmental disabilities, and/or substance abuse disorders. Employee will provide face-to-face, clinical assessment, diagnostic information, and recommended interventions (via treatment plan goals and objectives), based on client need, preference, and level of functioning. Employee will provide follow up services to ensure continuity of care and on-going clinical review of the provision of services for substance abuse services through review of client progress and clinical oversight of the ISP.
Essential Duties and Responsibilities
- Follows and ensures appropriate client care protocols; reviews client records, conducts thorough assessments confirming or revising client’s diagnosis, provides mental health and substance abuse services face to face with clients, reviews and signs the plan of care and periodically reviews the need for continued care.
- Provides individual, family and group counseling through the use of interviews.
- Provides substance abuse screening, evaluation, treatment and ongoing recovery services.
- Develops a plan of care for individuals related to their diagnosis, which must be a DSM-5 psychiatric and/or substance abuse diagnosis, and indicates goals related to symptoms and behaviors.
- Participates in staff meetings, treatment team meetings, and training activities to improve job skills and performance while ensuring care coordination.
- Maintains accurate documentation in accordance with applicable regulations to include progress notes, treatment plans, periodic reviews and other documentation related to patient progress and service delivery.
- Makes collateral contacts and ensures appropriate interaction with case managers and referring workers.
- Monitoring service delivery through contacts with clients receiving services and service providers to include site and home visits as necessary.
- Understanding the capabilities of services to meet the client's identified needs and preferences and to serve the client without placing the client at risk of harm.
- Performs other duties as assigned or required to make program successful.
Qualification/Skills
- Demonstrated knowledge, skills, and abilities in areas related to mental health, developmental disabilities, substance abuse, behavior management, family dynamics, developmental models, and community resources.
- Approach treatment within a person-centered, recovery-oriented framework, dealing with individuals with a range of moods and behaviors in a tactful, personable manner so as not to alienate or antagonize them.
- Ability to act independently in a clinical setting while incorporating administrative/supervisory feedback to ensure agency’s mission and values are upheld.
- Ability to diagnose mental health and substance abuse disorders, interpret data and facilitate treatment planning.
- Ability to lead treatment team meetings and coordinate care as needed.
- Exceptional written and oral communication skills.
- Exceptional interpersonal skills and demonstrated ability to establish relationships with patients, family members, coworkers, and community partners.
Education/Experience
Licensed or licensed-eligible mental health professional with 1-2 years’ experience working with substance abuse population. (Will consider CSAC level counselor with experience). Experience in utilizing ASAM criteria. Knowledge, skills, and abilities in areas related to mental health, developmental disabilities, substance abuse, behavior management, family dynamics, developmental models, and community resources. Thorough training and/or experience, demonstrated basic knowledge of behavior management, mental health, developmental disabilities, and substance abuse concepts, mental illness, and the provision of direct services to people living with mental illness.
Requires valid driver’s license. Must have successful completion of criminal history and central registry background investigation.

Pay: $37,000.00 - $46,000.00 per year
