Description:
The primary purpose of the Data Coordinator position is to complete administrative tasks related to referral review and admission processes within the referral management department. This role will ensure all patient information is available to the facility and support teams for each patient admission or readmission.
Responsibilities for Data Coordinator:
- Creates and completes patient profiles in electronic health record system, Point Click Care (PCC) for all new admissions, readmissions and returns from hospital including demographics, payer information and emergency contacts.
- Uploads all pre-admission documentation into PCC for new admissions, readmissions and returns from hospital including clinical documentation, payer verifications, background checks and other documentation.
- Identifies missing demographic information or documentation. Once identified, will complete verification or request from team members.
- Completes entry of all referrals into Customer Relationship Manager (CRM).
- Manages CRM by completing updates when referral status changes.
- Completes and distributes admission notifications to facility IDT once admissions are confirmed.
- Preforms other duties as assigned
- Participates in monthly weekend manager on duty rotation’s
Qualifications for Data Coordinator:
- 1 year prior SNF experience (preferred)
- High School diploma or equivalent
- High level of literacy in the Microsoft Suite (preferred) (Microsoft Office: Word, Access, Excel, E-mail, Outlook,.)
- Ability to read, write, speak, and understand English.
- Team oriented and cooperative
- Basic knowledge for Medicare/Medicaid and Managed Care Organizations
- Must meet all health requirements as required by law
