The Licensed Vocational Nurse (LVN)/ provides direct care for patients undergoing hemodialysis treatment, Home Assisted Hemodialysis Treatment and works under the direction and supervision of the Registered Nurse. The LVN demonstrates a basic understanding of contemporary and psychosocial nursing principles in relation to patients with chronic renal failure, adhering to regulatory and company policy and procedures.
Ø Licensed to practice as a Licensed Vocational Nurse as applicable to state laws.
Ø Successfully completed state mandated education and testing per state requirement
Ø A registered nurse or LVN with 18 months experience in hemodialysis obtained within
the last 24 months
Job Specific Accountabilities
The statements below describe the general nature and level of work being performed in this role. They are not intended to be a complete list of all duties, and additional responsibilities may be delegated as required, by the Director of Nurses or Registered Nurse.
Ø Performs timely and appropriate data collection and observations, responding with good judgment to observations and reports significant information to the RN.
Ø Performs dialysis equipment and machine functions prior to patient treatment, including safety checks, according to established policy and procedure. Performs pre-treatment evaluations as required.
Ø Initiates, monitors, and discontinues Hemodialysis, treatments in a timely manner. Evaluates patient vital signs before, during and after treatment. Successfully troubleshoots routine treatment issues and/or knows the appropriate resource to reference. Report out of parameter issues to supervising nurse. Document chart appropriately.
Ø Administers prescribed medications in a manner consistent with the knowledge of purpose, side effects, contraindications and established standards and protocols. Documents medications administered and patient’s response to medications when applicable.
Ø Provides complete and accurate documentation of all pertinent patient and facility data according to established policy and procedure.
Ø Implements and reinforces defined patient and care providers teaching plan as directed. Reports progress and/or concerns to the Charge Nurse or Director of Nurses. Recognizes and identifies educational needs and helps to mobilize appropriate resources.
