BASIC FUNCTION:
The Outpatient Services Coordinator will be responsible for providing recovery oriented group treatment, and clinical interventions where indicated, participating in monitoring attendance oversight and follow-up outreach where indicated. Additional duties may involve assistance with administrative and case management needs.
EDUCATION AND EXPERIENCE REQUIREMENT(S):
- Associates’ Degree required
- Credentialed Alcohol and Substance Abuse Counselor (CASAC) or Bachelor’s Degree in Human Services or related field preferred
- Experience in mental health and with addiction treatment highly desirable.
- Case Management experience preferred.
- Familiarity with Microsoft office and comfort level with IT interface ideal.

PHYSICAL REQUIREMENTS:
- Should be able to push/pull, lift/move 35 lb., be able to perform moderately difficult manual manipulations such as using a keyboard, writing, and filing for extended periods of time, must be able to perform tasks which require hand-eye coordination such as data entry, typing and using photo copiers. Mobility requirements may include the ability to be stationary at a workstation for a prolonged period time in addition to being able to squat or be mobile for a reasonable length of time and distance. Communication requirements include the ability to comprehend the spoken English language in addition to being able to communicate and read the English language.
PRIMARY RESPONSIBILITIES OF THE POSITION:
- Primarily provide group treatment (plan, coordinate, and facilitate therapeutic and rehabilitation oriented curriculum) and clinical services as needed.
- Participate in the development of group material (evidence based) curriculum. Prepare for groups in order to motivate client participation.
- Periodically review/evaluate/revise groups.
- Assume case management services according to client needs.
- Collaborate with team members for treatment plan development
- Participate in weekly treatment team meetings.
- Participate in clinical supervision.
- Coordinate with treatment team to generate referrals to groups
- Maintain updated group descriptions.
- Coordinate with Team the development and implementation of outreach activities:
- Utilize reports to identify individuals that have disengaged or at risk of discharge for low attendance to problem solve solutions to treatment.
- Assist primary counselors with outreach letters, tracking, and follow-up to retain current census when appropriate.
- Complete follow-up outreach post-discharge to comply with OMH Standards of Care.
- Maintain accurate tracking data for group member participation.
- Perform other duties as assigned
- Perform other assigned duties in a comprehensive and timely fashion.
- Provide back-up to front end/administrative staff as needed.
- Adheres to hospital policies and procedures related to:
- Mandatory Education: Continuing education records will be completed and forwarded to Human Resources annually regarding mandatory educational requirements.
- Annual Health Assessments: Complete annual health assessment form as required, maintains current inoculations.

Salary Range: $23.40-$33.93/hour Pay is based on experience, skills, and education. Exempt positions under the Fair Labor Standards Act (FLSA) will be paid within the base salary equivalent of the stated hourly rates. The pay range may also vary within the stated range based on location.
