BASIC FUNCTION:
Dedicated solely to the admission process; including, but not limited to, pre-admission visit documentation, admission, assessments, and development of the Individualized Recovery Plan.
EDUCATION AND EXPERIENCE REQUIREMENT(S):
- Minimum of CASAC, Bachelor’s Degree or above in human services preferred.
- Minimum 2 years of mental health experience and/or two years in addiction treatment preferred with an emphasis on diagnosing and assessing.
- Familiarity with Microsoft office and comfort level with IT interface ideal.

PHYSICAL REQUIREMENTS:
- Should be able to push/pull, lift/move 15 lb., be able to perform moderately difficult manual manipulations such as using a keyboard, writing, and filing for extended periods of time, must be able to perform tasks which require hand-eye coordination such as data entry, typing and using photo copiers. Mobility requirements may include the ability to be stationary at a workstation for a prolonged period time in addition to being able to squat or be mobile for a reasonable length of time and distance. Communication requirements include the ability to comprehend the spoken English language in addition to being able to communicate and read the English language.
PRIMARY RESPONSIBILITIES OF THE POSITION:
- Reports to PROS Psychiatric Supervisor and carries out duties as assigned by him/her
- Maintains effective tracking system utilizing various electronic medical records (i.e. AWARDS, MH App, Soarian, CAIRS)
- Completes Initial Screening, Screening and Admission Note, Attestation, Initial Service Recommendation, assists in completing the three required assessments and the Individualized Recovery Plan individually and in group format
- Participates in the prioritization and assignment of participants to clinicians in supervision and group meetings
- Acts as liaison to other community agencies in Intake-related issues
