What are the responsibilities and job description for the Chaplain position at Elm Management?
POSITION PURPOSE
Provide spiritual leadership to residents, families and associates in the community. This person will serve as a spiritual leader in each community and represent the community as the pastoral presence in the community.

ESSENTIAL FUNCTIONS
1. Know, understand, incorporate, and demonstrate the Mission, Vision, Values and Guiding Behaviors of Trinity Health and Trinity Senior Living Communities in behaviors, practices, and decisions.
2. Consistently demonstrate (leads by example) Sanctuary Values to all internal and external customers (residents, visitors, volunteers, and associates.) Create and maintain an atmosphere of warmth, propagating a calm environment throughout the community. Actively participates / intervenes with direct care as needed and as a role model in ongoing Sanctuary Training of personnel.
3. Execute assignments in a culture that is shared and collaborative across all divisions within TSLC.
4. Assists in establishing and/or maintaining departmental procedures that support the necessary operational functions for the Pastoral Care Department.
5. Assists in Maintaining department budget, place orders for equipment and supplies as necessary.
6. Assist in the recruitment, interviewing, and selection of personnel for the Community or Department.
7. Supports community personnel in home department and from other departments through coaching and mentoring to help achieve optimum standard of excellence. Addresses issues of concern through courageous conversation and notifies department manager of any interactions requiring attention.
8. Participate in community surveys and when necessary, develop a plan of correction for department deficiencies.
9. Arranges anointing of the sick on a regular basis or as needed. Networks with local clergy for the provision of the Sacraments and the celebration of the Eucharist as well as worship services for residents of other faith denominations.
10. Is respectful of the religious needs of all residents and provides for religious celebrations for residents of all denominations
11. Provides opportunities for corporate worship for residents, their families and staff. Examples of these may include: Good Friday Worship, reflection and sharing on scriptures and spiritual topics, healing touch, sing-along.
12. Provides opportunities for individual worship such as readings of scripture, prayers, reflections and spiritual reading.
13. Prepares Memorial Services for residents and staff quarterly or as requested. Prepares for a ritual that notes each person’s passing and allows residents and staff to grieve.
14. Review chart to become familiar with advanced directives and check for immediate spiritual needs within 7 days
15. Completes a spiritual assessment with outcomes and resident or family participation within 7 days of admission to long-term care.
16. Participates as a member of the residents’ interdisciplinary team, he or she participates in the resident’s initial and updated care planning as well as process improvement.
17. Care plan reflects coordination of services with other parallel disciplines such as social services and activities.
18. Participates in meetings of Administrative team, Units as needed and department and community committees.
19. Serves as an advocate for staff at management meetings
20. May conduct in-services to create awareness of death and dying issues, grief and diversity sensitivity training. May provide information and education for residents’, responsible persons and staff in the area of Advance Directives
21. Understands the different aspects of grief: anticipatory, disenfranchised, acute, chronic and compound.
22. Creates plans to address the different stages of grief a resident/family/staff may experience.
23. Lets staff, family and residents know that he/she welcomes people to discuss their feelings of loss, leads resident support groups.
24. Offers opportunities for discussion of complicated medical ethical issues. May facilitate in-house ethics committee meeting and may participate in regional ethics committee meeting.
25. Dialogues with residents and their families on issues of end of life, the withdrawal/termination of life support, comfort measures, informed consent and refusal of treatment
26. Knowledgeable of the current Ethical and Religious Directives for Catholic Healthcare services
27. Supports the Standards of Excellence for Spiritual Care of Trinity health System.
28. Provides spiritual activities for residents
29. Must function independently, make independent decisions, demonstrate flexibility, personal integrity and ability to work effectively with residents and personnel.
30. Report occupational exposures to blood, body fluids, infectious materials, and hazardous chemicals in accordance with the community policies and procedures.
31. Maintain a working knowledge of applicable Federal, State, and local laws and regulations, TSLC Corporate Integrity Program, Code of Ethics, as well as other policies and procedures in order to ensure adherence in a manner that reflects honest, ethical and professional behavior.
MINIMUM QUALIFICATIONS
1. Bachelors Degree, satisfactory completion of at least 3 units of clinical pastoral education at ACPE or USCB approved centers or at least 5 years experience either acute, sub-acute, long-term or hospice settings.
2. Previous experience working in long-term care or with the geriatric population preferred
3. Supervisory experience preferred.
4. Must lead by example and possess superior customer service skills and professionalism
5. Flexibility to adapt to ongoing change and work in a fast-paced, customer driven environment.
6. Interpersonal skills to drive collaboration, commitment and productivity when working with cross-functional teams, customers and end users.
7. Self-starter, highly motivated with high energy level.
8. Demonstrates superior written and verbal communication and presentation skills.
9. Basic to intermediate proficiency with Microsoft product suite (MS Word, Excel, Power Point, etc.); basic knowledge and experience with electronic mail and calendaring system in PC LAN environment. Ability to type with speed and accuracy.
10. Possesses a high degree of personal accountability, responsibility and independent decision making abilities with the skills to plan, organize, develop, implement and interpret programs, goals, objectives, policies and procedures of the organization.
