What are the responsibilities and job description for the CNA or HHA PRN position at Empath Home Health?
Primary tasks, duties, and responsibilities:
- Promotes and practices Avidity’s mission and values and follows policies and procedures.
- Maintains confidentiality of patient information and all Avidity operations.
- Provides patient care in compliance with agency policies and procedures and federal, state and local regulations.
- Assists patient with personal care services such as:
- Personal hygiene
- Bathing
- Grooming, dressing, and toileting
- Transfer, ambulation, and exercise
- Assists patient with medications that are self-administered if certified.
- Reports changes in patient condition in a timely manner to the supervising Registered Nurse or Clinical Manager.
- Provides patient with nutritional support by preparing meals and feeding, if necessary and records intake and output of nutrition and fluids.
- Assists patient with range of motion and positioning techniques.
- Takes patient’s vital signs including temperature, pulse, respirations; may be asked to record patient’s weight.
- Maintains a safe and clean environment in patient’s home.
- Documents skin condition at every visit notifying patient’s Registered Nurse or Clinical Manager of any changes.
- Identifies patient with two identifiers: one at the first visit and one subsequently.
- Documents patient care accurately on visit notes.
- Utilizes proper body mechanics during provision of patient care.
- Notifies the Registered Nurse or Clinical Manager of inadequate supplies or malfunctioning medical equipment.
- Maintains effective working relationships with clients, staff, representatives of other departments and community agencies.
- Participates in staff meetings and patient case conferences as requested.
Educational/Professional:
1. High school diploma or equivalent required.
2. Minimum of one (1) year experience in home health care.
3. Home Health training to include Concept Differences in Families, Household Management, Role of the HHA, Ethics and Conduct and food management required.
4. Has met the minimum forty (40) hours required by state and federal regulations or the HHA competency exam.
5. Successful completion of HHA competency evaluation.
6. Additional training by an accredited vocational school or hospital or currently enrolled in an accredited School of Nursing with six (6) month’s clinical experience preferred.
7. An equivalent combination of professional experience and education may be considered as meeting the educational/professional requirements.
8. Current CPR certification.
