About Us:

Responsibilities:
- Develop meaningful relationships through remote interactions with members and their families, understanding their unique needs and preferences.
- Implement Person-Centered Service Plans (PCSP) and advocate for positive changes in PCSP meetings.
- Conduct interviews for Direct Support Professionals, ensuring an ideal match based on individual needs.
- Monitor service quality, providing guidance and support to enhance the client and provider experience.
- Manage relationships between providers, clients, care teams, and the organization for mutual success.
- Ensure accurate billing and timely service delivery.
- Educate prospective DDD members on the benefits of HCBS services.
- Participate in company events, meetings, and maintain strong community partner relationships.
- Stay compliant with state DDD & AHCCCS policies through regular review and attendance at relevant meetings.
- Other duties as assigned.
Requirements:
- Previous experience in roles such as HCBS Coordinator, Care Manager, Group Home Manager, or Direct Support Professional.
- Ability to work independently and collaborate with Phoenix headquarters remotely.
- Reliable home internet service
- Genuine passion for the special needs community and a commitment to making a positive impact.
- Knowledge of Developmental Disabilities programs and service systems in Arizona.
- Proficient skills in Case Management (Interviewing, Advocacy, Collaboration).
- Ability to maintain confidentiality with knowledge of HIPAA.
- Proficiency in computer skills, familiarity with Apple products & Google Drive.
- Valid Level One fingerprint clearance card.
- In-state travel required; must possess a valid ARIZONA state-issued driver's license and access to personal transportation (mileage reimbursed).
- Subject to a search of both the Child Protective Services Central Registry and the Adult Protective Services Registry.
