Position Highlights
- Position: Community Health Worker (CHW)
- Location: Evanston, IL
- Full Time/Part Time: Full Time
- Hours: Monday-Friday, day shift. In addition, there may be occasional evening or weekend hours for community events.
- Travel: Frequent local travel will be required. You must either maintain a valid State of Illinois Driver’s License with proof of active insurance or have access to other means of transportation.
- Assess and address patient’s needs in the area of housing, food insecurity, transportation or other non-medical areas that influence a patient’s health outcomes (Social Determinants of Health).
- In partnership with patient care teams, provides support for patients in gaining access to medical services, screenings, prescription medication and durable medical equipment.
- Assist patients with getting access to social services and other community-based resources.
- Follows up with patients via phone calls, home visits and visits to other settings where patients can meet.
- Maintain accurate documentation of patient contacts and document patient’s needs, action plan and outcomes in the medical record system.
- Represents the organization at various community events.
- Participate in continuing education, trainings and CHW organizations.
- Education: High school diploma or GED
- Experience: Minimum of 2 years of community based experience.
- Skills:
- Strong passion, leadership and interpersonal skills necessary to effectively work with multi-cultural, diverse populations.
- Ability to effectively engage in conversations, motivate and encourage patients to take steps to maintain and improve their health.
- Creative thinker with ability to problem solve and brainstorm solutions.
- Ability to work collaboratively as a part of a patient care team.
- Ability to work independently with minimal supervision.
- Leadership and community education experience.
- Basic computer skills required. Experience with EPIC EMR preferred.
- Fluency in Russian, Spanish, Urdu, Hindi, or Arabic a plus.
- Certification: Community Health Worker, Health Coach, Patient Advocate Certification and/or equivalent preferred.
