JOB PURPOSE: JOB PURPOSE:
The Case Manager is responsible for assessing the needs of unaccompanied children in care, developing individual service plans, screening for human trafficking concerns, facilitating the safe and timely release or discharge of children and youth, and documenting the provision of services in the case files. The Case Manager will work primarily to help unify unaccompanied children who are residing at Pecos Children’s Center in Pecos, Texas.
ESSENTIAL JOB RESPONSIBILITIES:
- Oversees all aspects of the Unaccompanied Child’s (UC) case and provides release recommendation for safe and timely release.
- Maintain case file documentation in accordance with ORR requirements.
- Maintain confidentiality when gathering, documenting, consulting or consolidating information necessary for the case management assessment and the plan of care.
- Maintain regular contact with each UC, UC’s family when possible, and established service providers to communicate case planning and progress.
- Provides client services as needed, including but not limited to making medical appointments, finding resources, etc.
- Gather historical and current data relative to completion of a comprehensive case management assessment including, but not limited to, school records, medical records, mental health treatment, legal status and court involvement, social and family strengths and needs, support systems, and specific cultural issues.
- Presents case to federal stakeholders as requested.
- Participate in team meetings, staff in-service training and department meetings.
- Carries a case load of up to 12 clients, varying on program census.
- Completes Signification Incident Reports (SIRs), per federal policy requirements.
- Conducts internal chart reviews to monitor documentation compliance.
- Completes, timely and accurate timesheets and other personnel/administrative records as required.
- Other duties as assigned.

ESSENTIAL QUALIFICATIONS:
EDUCATION: Bachelor’s degree in the behavioral sciences, human services or social services fields.
EXPERIENCE:
- Bi-lingual in Spanish and English, written, read and verbal (required).
- Attained age 21 years or older before first day of employment in position (required).
- ORR Experience preferred
- Knowledge of Federal, State and agency regulations preferred
- Knowledge of adolescent developmental issues, trauma, abuse and neglect, substance abuse and
dependence, delinquency, peer influence, and related group and individual interventions preferred.
ATTENDANCE: Must maintain regular and acceptable attendance at such level as is determined in the employer’s sole discretion.
LICENSES: Driver’s License with clear record.
VEHICLE: Must have daily use of a vehicle without prior notice.
OTHER: Must be available and willing to travel to various locations and with such frequency as the business need dictates. Must not pose a direct threat or significant risk of substantial harm to the safety or health of himself/herself or others.
EXCEPTIONS TO THESE CRITERIA MUST BE APPROVED BY THE DEPARTMENT HEAD.
