Schedule: Monday – Friday, 8:00am – 5:00pm

The RN Care Management Team Lead is responsible for assisting in maintaining all program clinical and logistic responsibilities within the care management department.
- Assist in maintaining all program clinical and logistic responsibilities within the care management department
- Assist with scheduling, tracking, trending, and reporting on key operational and quality metrics
- Collaborate with departmental leadership, strategic partners, and local branch staff to achieve the goals of the quadruple aim as set by the centers for Medicare and Medicaid services (CMS), and the needs of the department.
- Provide registered nurse (RN) oversight when licensed vocational nurses (LVNs) develop patient care plans
- Function as the leader for care management clinical best practices, keeping up with new CMS rulings, requirements and best practices
- Participate in telephonic quality audits and provide coaching to staff to improve quality of care
- Carry a caseload of patients to provide care management to
- Monitor the education needs of the team and provide needed education
- Interact with home health branches to educate on the importance of high risk patients participating in IVR calls to lower readmission rates for the branches
- Must be a Registered Nurse (RN) with licensure in the State of Texas.
- Must have at least 3 years demonstrated experience in care management best practices in a telephonic care management setting, health care setting, such as a clinic, hospital or post acute facility.
- Must have at least 1 year of team lead experience.
- Must have intermediate to advanced skills in various software, especially Excel and PowerPoint
- Must have strong skills utilizing various Electronic Medical Record (EMR) platforms.
- Must have demonstrated execution in effective problem solving, escalating issues according to procedure.
- Case management certification highly preferred.

Previous experience and knowledge of patient risk management software is highly preferred.
- May include experience in Medalogix, Innovaccer, Epic Systems, or Eclinical Works.
