Envision is looking for a dedicated individual to become a part of our Case Management team as an Intake & Family Support Case Manager. This is a temporary, full-time position with Envision (estimated 3 months) and has the opportunity to become a long-term full-time position with the new contracted case management agency.
SHIFT: Full Time Position - 36 hours/week, Mon-Thurs, 7:30 am - 5 pm
Enjoy a 4-day, Monday-Thursday, 36 hours per week work schedule with occasional evening and Friday hours (due to family schedules). Case Managers have the opportunity to work remotely 2 days a week once they are trained and working independently, with the other 2 days being in office.
PAY RATE: $19.84 per hour

SUMMARY:
The Intake and Family Support Case Manager will work to enhance the quality of life for people with intellectual and developmental disabilities by assisting eligible people to enroll into services, and to monitor the services provided to ensure their needs are being met. The Case Manager will assist the individuals and families supported to connect with community services and develop and maintain natural supports.
Case Managers help people with intellectual and developmental disabilities and their families to navigate all of the different types of services that may be available to them. Case Managers take families through the intake process to determine eligibility; discuss the different services and programs and help families choose the best fit; determine the needs of the eligible individual; work together with the individual and families to develop a Service Plan; identify other programs and services outside of the developmental disabilities system that may be of help to individuals; and provide ongoing monitoring of services being provided.
