JOB SUMMARY
The CAS Assessor is responsible for arranging, conducting , and completing accurate and thorough assessments using the person-centered approach .
JOB RESPONSIBILITIES
- Outreach to individuals and/or their legal gua r dians o r ca r etake r s as necessary du r ing the assigned schedule
- Schedule and coordinate the assessment process with individuals, families , and providers in the home and/or community using a person-centered approach
- Schedule and conduct the assessment at a location and t i me which is most convenient for the individual.
- Utilize an in-person , pe r son - centered interview process as part of the assessment pro c es s ( e . g. conversational style that starts with the person)
- Conduct additional interviews with the knowledgeable person ( s) and utilize multiple sources i ncluding records to verify information and coding prior to finalizing the assessment.
- Work with Operations Support to arrange formal i nterpreter services as needed.
- Complete all assessments utilizing the provided softwar e.
- Code the assessment accurately based on t r aining.
- Document all case activity as per program requirements.
- Meet or e x ceed established monthly productivity standards.
- All assessments must meet quality standards ( e . g . assessment fully completed , individual pa r ticipated in the assessment , time frames of assessment maintained , etc. )
- Perform other duties as may be assigned by Management.
