EDUCATION AND/OR EXPERIENCE
Registered Nurse credentialing the State of New Jersey is required. Must maintain a license in good standing. At least two (2) years of outpatient experience. At least one (1) year of clinical nursing experience with familiarity in the use of electronic medical records (EMR). Demonstrated ability to provide culturally competent health education to patients and families. Demonstrated ability to assess patients over the phone, accurately triage non-urgent and urgent patient calls, and provide accurate and safe telephone advice is required. Effective oral and written communication skills are required. Ability to function in a team environment is required. Basic word processing and computer skills are required.
Credentials Required: New Jersey Professional Nurse License and Registration; ACLS and BCLS.

ESSENTIAL DUTIES AND RESPONSIBILITIES
- Assesses the health status of assigned patients as outlined in the nursing standards.
- Provides and documents direct patient care within scope of practice to ensure implementation of the plan of care. Plan of care will be communicated to the physician and other members of the health care team in a timely manner.
- Conducts field work to initiate patient medical assessments including the collecting and documenting of vital signs and or administering a TB test in the form of a TST or IGRA.
- Conducts TB interviews and contact investigations while properly documenting all field work to ensure appropriate follow-up with cases, contacts, and interjurisdictional and immigration referrals for the prevention, treatment and control of TB transmission.
- Informs and instructs patients/significant others about patient's health/illness state and plan of care in collaboration with members of interdisciplinary team.
- Observes and monitors patients' status and safety. Delivers physiological and interpersonal care according to standards consistent with the multidisciplinary treatment plan, nursing care directives, and expected outcomes.
- ensures all procedures are performed based on established standards; demonstrates competency in performing all procedures within scope of practice.
- Prepares and administers medications as prescribed by the physician or APN and parenteral therapies in accordance with established policies and procedures.
- Prepares and submits timely and accurate NJDOH surveillance forms.
- Assists physicians during scheduled office hours including preparations and setup of clinic exam rooms for patient visits. Reviews and prepares patient charts prior to office visits for essential tuberculosis paperwork and test results.
- Performs standard diagnostic procedures including EKG, phlebotomy, venipuncture, and ensure proper infection control standards are maintained. Prepares and protects specimens for transport.
