Title: Clinical Pharmacy Technician
To support the Clinical Pharmacy team workflow and provide direct telephonic patient care to support overall quality of Comprehensive Medication Management services and Pharmacy related quality metrics for our high risk attributed patient population.
The Clinical Pharmacy Technician will be expected to demonstrate ability to perform the following:
- Gather data and generate reports to improve clinical pharmacy quality metrics.
- Document patient cases in electronic medical record effectively.
- Appropriately identify situations for referral to Clinical Pharmacist (i.e. medication adverse event, medication ineffective, drug-drug interaction, lab test indicated).
- Reinforce patient self-management of medications through provision of education on proper medication administration, medication adherence, storage, and drug disposal.
- Adhere to all Summit Health Management training and accomplishments as required per policy.
- Provide other job duties as assigned.
- Report issues and concerns to manager in a timely manner.
- Based on chart review or patient interview, identify patient specific reasons for poor medication adherence and take appropriate action to resolve problem.
Education, Certification, Computer and Training Requirements:
- Associate’s or Bachelor’s Degree
- Board of Pharmacy Technician registration
- Certified Pharmacy Technician (CPhT) with PTCB
- Minimum 3 years of retail Pharmacy practice
- Ability to communicate in English, both orally and in writing
- Standard Office Equipment (Phone, Fax, Copy Machine, Scanner, Email/Voice Mail)
- Standard Office Technology in a Window based environment
- BLS Certification
