Exceptional Care & Training Center is a Medically Complex facility for persons with Developmental Disabilities (MC/DD). We are looking for compassionate, responsible Qualified Intellectual Disabilities Professionals (QIDPs).

We are looking for a full time QIDP / Case Manager!

  • RN or Bachelor Degree in professional health related field as defined by Illinois QIDP Registry and at least one year of experience working with persons with intellectual disabilities required.

ESSENTIAL DUTIES AND RESPONSIBILITIES: An individual must be able to perform each essential duty satisfactorily.   • Educates and promotes understanding of social and emotional factors underlying resident health conditions. Responsibilities include but are not limited to: collaborating with physicians/health care personnel regarding resident treatment and evaluation; assisting residents, families and legal representatives regarding medical recommendations, consent, changes in condition, and to encourage family participation in developing plan of care.
  • Maintains social service and functional assessments. This includes but is not limited to: completion of initial as well as ongoing social service assessments, as part of the Comprehensive Functional Assessment (CFA) process; regularly updating and auditing assessments; ensuring assessments identify residents in need of social service interventions.
  • Oversees administration of Individual Program Plan (IPP). This includes but is not limited to: working with departments to meet the needs of each resident; regularly reviewing services to determine whether IPP addresses resident needs; evaluating the progress of each resident; developing residents’ daily, weekly, and monthly schedules, maintaining contact with resident guardians and encouraging participation in the plan of care.
  • Promotes optimum resident health. This includes but is not limited to: providing services planned to restore each resident to his or her optimum social and health adjustment for his or her capacity; arranging for mental health services to attain or maintain the highest practicable mental and psycho-social well-being; identifying and care planning ways to accommodate residents’ choices, preferences, and customary routines; communicating information to direct care staff, supervise and evaluate active treatment, perform daily rounds of the Center to ensure provision of active treatment.   • Assists with admission and discharge processes. This includes but is not limited to: assisting with preadmission and care planning for admissions, evaluating residents for discharge potential, provision of discharge planning services to reflect resident and family preferences for care, coordination of post discharge care and services, coordination of transportation, access to and payment for services. Prepares discharge summary for release to authorized persons and agencies.
  • Promotes interaction with the community. This includes but is not limited to: using community resources to assist resident to resume life in community, and/or to learn to live within limits of physical and mental ability; using community resources to assist in resident care; recommending appropriate types of services to residents; implementing a system to preserve resident choice and access regarding community resources; and providing in-service training for staff and the community.
  • Participates with Executive Director/designee in survey processes. This includes but is not limited to: assisting with survey readiness; instructing staff regarding conduct and disclosure; being present at all times surveyors are on-site; directing prompt responses to requests for information; collaborating to develop responses to survey report as needed; and undertaking corrective action.
  • Works cooperatively with members of the Interdisciplinary Team (IDT). This includes but is not limited to developing, implementing, and evaluating plan of care, leading care conferences, communicating resident concerns and responses to interdisciplinary team members of direct care staff, serving on committees, as directed.
  • Effectively coordinate internal and external program services and supports to facilitate the acquisition of client skills and adaptive behaviors; and promote competent interactions of staff with clients in program implementation and behavior management.
  • Observes safety practices. This includes but is not limited to: assisting with work place safety compliance; promptly and appropriately reporting potential safety concerns, incidents, and/or injuries; participating in fire and disaster drills. In an emergency, carries out assigned duties to assure resident and employee safety, perform daily rounds of the Center to inspect care environment.
  • Performs quality assessment and assurance functions and ensures compliance with the standards established by the quality assessment and assurance program.
  • Timely and accurately maintain proper documentation. This includes but is not limited to: recording progress notes in the clinical record, audit clinical records.
  • Assists with development and update of policies and procedures to reflect the philosophy of the facility, professional standards, and legal requirements.
  • Must complete a DHS-approved QIDP orientation training program within six (6) months of hire.
  • Maintains and applies current skills and knowledge through continuing education and in-service programs. After being added to the state QIDP database, The QIDP orientation training program cannot be completed in less than 21 calendar days, but once started, it must be completed within 120 calendar days.
  • This position requires excellent interpersonal skills with high level of energy, enthusiasm, flexibility in work schedule; good verbal and written communication skills; and the ability to organize, document, and implement detailed programs. Must be able to speak, understand and read the English language to the extent necessary to safely and properly care for residents. Must maintain a sense of professionalism in regard to dress, language, and conduct.
  • This position requires standing, sitting, stooping, pushing, pulling, lifting up to 50 pounds or more, bending, climbing, twisting upper body, walking, running, carrying, listening, talking, use of facility telephone, and exposure to medical equipment and persons with risk of illness
