UnityPoint Health – Eyerly Ball is seeking a full-time, non-exempt Case Manager for our Assertive Community Treatment Team (ACT) in our Des Moines office.

As a Case Manager, you will work directly with clients by following an evidence-based practice designed by NAMI to provide comprehensive and wrap-around services to help clients achieve mental health, physical health, housing, employment, social goals and other goals as identified by the client.

As a Case Manager you will:

• Assess the needs of clients and develop goals that will allow clients to achieve the greatest level of independence in their community.
• Be part of the team that teaches skills to clients to creatively explore alternatives to hospitalization, higher level of care, homelessness and incarceration.
• Advocate for clients to help meet their needs, desires and best interests.
• Utilize critical thinking in making independent judgements and maintain responsibility and accountability to our clients.
• Ability to meet clients in their homes or in public places with ability to drive safely and a willingness to transport clients in own vehicle.
• Obtain medication management certificate within first three months of hire to be able to administer medication to clients.

Ideal Candidate:

• Capacity to professionally relate to a wide variety of clients, as well as a desire to serve as a client advocate.
• Ability to practice good verbal and written communication skills with ability to complete case notes timely.
• Capacity to communicate with clients effectively, patiently and respond professionally at all times.
• Ongoing passion about making a difference in the lives of those who have been touched by mental illness.

Requirements

To be considered you’ll need:

• Bachelor’s degree in human services or related field.
• One year with population served.
• Capacity to obtain medication management certificate within three months of hire.
• Possess a valid driver’s license.
• Required to provide proof of auto insurance coverage.

Schedule:

• Monday-Friday, 8:00 am to 4:30 pm, plus occasional weekend and evening hours.

COVID-19 and Influenza Vaccines:
