(RN) Registered Nurse - Home Care

Family Bridge Healthcare Services, (FBHS) is a Medicaid Certified Private Home Care agency hiring for a Registered Nurse (RN), part-time in office and in-field.

• Home care experience, medicaid waivers program knowledge, preferred
• Leadership skills and accuracy in completing tasks,
• Supervising team members,
• Strong Communication skills and attention to detail

Duties and Responsibilities: (Perform other duties as assigned)

• Observe and monitor individuals plan of care according to the physician’s instructions.
• Works independently and as a team.
• · Review Medication Administration Record (MAR), Individual Service Plan (ISP) documents for tracking, individual’s medication, and doctors’ orders to ensure compliance at least monthly or whenever there is change in condition of the individual’s health and/or environment.
• Collaborating with schedule coordinators and office staff
• Traveling about 25% of the time; completing home visits within the surrounding Atlanta area.
• Coordinate assessments and implement care plans
• Assist and support CEO/COO with daily operations.
