Family Health Centers of San Diego is looking for a Referral Specialist (PACE) who will work collaboratively with the PACE clinic to schedule participants appointments. Referrals will be processed timely and accurately.
Responsibilities:
- Accurately completes referrals to include ICD-9 and CPT codes, following established department protocols. Makes initial appointments directly into CMIS; gives appointment cancelation information.
- Actively tracks follow-up and referral pool(s); schedules patient services as required.
- Communicates to clinic staff status of pending referrals using CMIS updates and chart notes; indicates referral status changes as they occur.
- Enters required data, including basic insurance and screening information.
- Follows established protocols for patients who indicate they are in crisis.
- Makes assigned reminder calls.
- Submits accurate and timely data about referrals and appointments by site.
- Performs other duties as assigned.
Requirements:
- High school diploma or GED equivalent required.
- Successful completion of a Medical terminology course preferred.
- 1 year of experience in an ambulatory care setting required.
- Or equivalent combination of education and experience that provides the skills, knowledge and ability to perform the essential job duties, and which meets any required state or federal certification requirements.
- Bilingual in Spanish/English preferred; may be required in some positions.

If you are hired at Family Health Centers of San Diego, your final base salary compensation will be determined based on factors such as geographic location, jurisdictional requirements, skills, education, and/or experience. In addition to these factors – we believe in the importance of pay equity and consider internal equity of our current team members as a part of any final offer. Please keep in mind that the range mentioned above is what we reasonably expect to pay for the role. Hiring at the maximum of the range would not be typical in order to allow for future and continued salary growth.
