Our client, a Non-Profit Healthcare Organization in Hartford, CT, is in need of a Registered Nurse for a full-time position working with their Clinical Response Team.

RN Minimum Requirements:

  • Bachelor’s degree in nursing services
  • 2 or more years of experience in the residential agency setting or the DHMAS/Psychiatric nursing field.
  • Bilingual English and Spanish, both written and spoken.
  • At least age 18
  • Valid driver’s license

About the position:

Under the direction of the Clinical Supervisor and the RN Supervisor, The Clinical Response Team (CRT) Registered Nurse works 20 hours a week: Monday thru Friday and works with the Clinical Response Team to facilitate the assessment and planning of all health needs and services for individuals referred to the Clinical Response Team. Facilitates and follows the directions of medical providers to maintain individuals’ health and well-being in keeping with nursing standards, Department of Developmental Services/Department of Public Health (DDS/DPH) regulations and mission.

RN Responsibilities:

The Clinical Response Team RN works in cooperation with the Clinical Response Team Coordinator and Response Team Clinician, reporting to the Clinical Supervisor. While this position does not include direct employee supervision, LPNs and direct care employees may work under the direction and license of the RN with regard to medical matters.

Works with the other CRT members to assess the needs of individuals referred to the CRT with a focus on medical matters. Subsequently works with the other CRT members to plan for and transition the referred individuals to appropriate living and work placements as needed, ensuring that medical needs are met. Works under the direction of the RN Supervisor for clinical nursing supervision, and under the direction of the Clinical Supervisor administratively. The CRTRN may review cases through the CRTs area of responsibility, including the DDS North and South Regions, with possibility that there may be some cases in the West Region.

Research and, if necessary, secures medical, dental, pharmacy and related health care services in the community, and works with health professionals maintaining regular contact. Also works with families and vendors in order to remain informed of changes which might affect services for individuals.

Communicates problems and issues relating to the health of individuals and follows up in writing until case closure to ensure that these are addressed in a timely manner. Represents in a positive and professional manner when interacting with individuals, co-workers, families, outside agencies and others in the community. Ensure that all actions, activities, and decisions are aligned with vision and values. Creates a welcoming, attractive, person-centered, culturally aware, and respectful environment for individuals and staff.

Advises/assists identified service recipients with medical/health-related appointment schedules (regular appointments as well as referrals/recommendations.) Prepares appropriate forms and documents follow-up to facilitate the maintenance of individuals' health. Accompanies individuals on special medical appointments when necessary.

Participates in Clinical Rounds, CRT case reviews, and Planning and Support Team meetings regarding pre-admission, transition, and discharge planning to exchange information pertinent to individuals physical and mental health.

The CRTRN will assess and monitor the health status of individuals on an on-going basis, following the orders of the physician, with the assistance of each individual current and possible future support team members as a means of ensuring optimal health. Assesses and reviews the nutritional status of each individual and consults with a dietitian, if necessary, to ensure that individuals dietary needs are met.

Interacts with individuals on an on-going basis to better assess and understand their needs, likes, dislikes, and personalities, while developing familiarity with their individual health issues, thereby providing better, personalized service.

As appropriate, assists in developing and monitoring self-administration of medication program as directed by each individual’s physician, to ensure that individuals are allowed the opportunity to develop self-reliance in the taking of their medication.

Fully utilizes electronic health records to review, document and monitor the individual’s health. Creates health care summaries and submits these to the Planning and Support Team prior to transition meetings to ensure input on health-related issues. Reports communicable diseases to the CRT members and RN supervisor so that proper prevention measures can be taken.

Teaches support staff and individuals in health-related issues including safe storage, handling and administration of medications; health care needs, and personal care / hygiene; in order that they have the information and delegation required to ensure that individuals maintain good health.

Reviews and initials all incoming medical/health related information including physicians'/dentists' notes; lab reports, and any special tests, to maintain awareness regarding such information. Follows up with medical professionals as necessary. Requests and receives nursing discharge summary and physicians’ orders when an individual is discharged from the hospital to ensure that support staff and supervisors are aware of the type of follow up care needed.

Monitors and assesses available medication administration, incident reporting, restraint and other health related reports, following up with Planning and Support Team members, to ensure that appropriate measures are taken where necessary.

Performs other duties as assigned by the RN Supervisor or Clinical Supervisor.

Pay: $25.50 - $33.75 per hour
