Social Work Assistant, PACE

Family Health Centers of San Diego is looking for a Social Work Assistant, PACE to work under the direction of the MSW Lead, the Social Work Assistant is responsible for direct case management services to PACE participants. Repsonsibilities: In conjunction with the IDT, may meet with family members and others. Coordinate ongoing family meetings, as needed. Provide ongoing case management and advocacy as required. Completes all required documentation accurately, in a timely manner, and thoroughly in accordance with department standards. Assists in preparing reports as required. Review care plans and SMART goals with clients; clients are high-need and require more intense services. Provides intensive individual support to high need clients. Support may include providing therapeutic interventions, providing internal and community services referrals, and more intensive support may include accompanying clients to housing services, appointments, social services, etc. Focus on medical adherence, support, and appointments designed to maintain necessary care. Coordinate and schedule assessments for MSW’s/RD’s/RT’s/PT and OT. Function as liaison between the participant and other agencies such as Department of Aging, Social Security Administration, Medicaid, etc. Assist education on budgeting and financial awareness with participants. Assist with ongoing financial eligibility for participants, including recertification as needed. Maintain confidentiality of participant information. Attend and participate in staff meetings, in-services, projects, and committees as assigned. Adhere to and support the center’s practices, procedures, and policies including assigned break times and attendance. Maintain caseload and follow up for ancillary services. Assist MSW’s in following through of participant care plans. Maintain open communication with MSW’s for ongoing support. Performs other duties as assigned. Requirements: Bachelor’s degree in social work or related field strongly preferred or minimum 2 years work experience providing human services to high risk medically underserved population required. Ability and means to travel as needed in a timely manner within San Diego County, to locations that may have limited access to public transportation; proof of liability and property damage insurance on vehicle used is required. DRIVER REQUIREMENTS: Age 21; Licensed for a minimum of 3 years; No violations and/or accidents within 3 years; No vehicle related suspensions/reinstatements; No DUI, reckless or felony Driving within 7 years. Bilingual in English/Spanish preferred. One (1) year of documented experience in working with a frail or elderly population preferred. Experience in a community-based setting or geriatric program preferred. Interest in the risk-based long-term care program to serve frail elderly in a community-based setting. Experience and thorough knowledge of social service principles and practices. Knowledge of psychosocial, behavioral, and family needs of the elderly population. Knowledge of financing mechanisms such as Medicare, Medicaid, and Prospective Payment Systems. Knowledge of the local and social service delivery systems and aging network. In the spirit of pay transparency, we are excited to share the base range for this position, exclusive of fringe benefits or potential bonuses. $24.00 - $29.97 If you are hired at Family Health Centers of San Diego, your final base salary compensation will be determined based on factors such as geographic location, jurisdictional requirements, skills, education, and/or experience. In addition to these factors – we believe in the importance of pay equity and consider internal equity of our current team members as a part of any final offer. Please keep in mind that the range mentioned above is what we reasonably expect to pay for the role. Hiring at the maximum of the range would not be typical in order to allow for future and continued salary growth.
