What are the responsibilities and job description for the Prevention Services Coordinator position at Fond du Lac County?
The ADRC Prevention Services Coordinator, provides prevention services as directed through the annual ADRC contract. The coordinator will research and implement Health Promotion, Prevention, and Early Intervention programming for the Aging and Disability Resource Center of Fond du Lac County. This position provides various Health Education services to residents of Fond du Lac County, through preparation and delivery of educational programs along with marketing and outreach services related to the Aging and Disability Resource Center. This position participates in marketing and public education, developing promotional materials and presentations and conducting prevention and outreach activities to suit the needs of the community. The following duties are normal for this position. These are not to be construed as exclusive or all inclusive. Other duties may be required and assigned as best practice or requirements change within the social worker practice field.
- Provide prevention services as directed by the annual ADRC contract.
- Attend and successfully complete training in state-approved evidence based health promotion programs.
- Prepare/Organize and conducts educational and prevention programs in group settings or one-on-one.
- Prepares reports.
- Develops and maintains communications with local human services agencies, businesses and others as necessary for health education programming.
- Prepares publications, i.e., news releases about health programs in the community, including articles for the ADRC quarterly newsletter.
- Participates as a liaison with the media including radio and other media activities.
- Represents the Aging and Disability Resource Center at appropriate meetings and committees.
- Prepares and updates appropriate health education curriculum for target populations.
- Participate in Quality Assurance/Quality Improvement projects and activities.
- Complete administrative duties in a timely manner, including, but not limited to: Individual client documentation via computerized files (Wellsky), required agency paperwork, including 100% time reporting, payroll, research projects and other records for departmental functioning. This would be accomplished through application of organizational skills and time management techniques.
- Establish and maintain a working knowledge of the responsibilities of various community, county, state and independent resources available to agency clients and the ability to utilize these resources as appropriate.
- Edit and keep ADRC website and social media accounts up to date to maintain accuracy and proper marketing.
- Coordinate and oversee all trained leaders and other staff or volunteers associated with ADRC health promotion programming. Recruit leaders/volunteers and establish a “falls prevention” committee.
- Intake phone coverage.
- Performs other duties that relate to the overall promotion of preventative health opportunities for ADRC target populations or other relevant ADRC activities as assigned.
- Demonstrated ability to exercise sound judgment and discretion in applying and interpreting programs and procedures.
- Competent Computer Skills and computer experience.
- A demonstrated ability to empathize with agency clients.
- Must possess a valid driver’s license

- Auto insurance consistent with the Fond du Lac County fleet policy
- Willingness to utilize personal vehicle as necessary to perform job duties and responsibilities
- Graduation from an accredited college or university with a Bachelor's Degree in Community Health Education, Public Health, Human Services, or other suitably related field.
- Experience working in health promotion, prevention, early intervention.
- Experience working with target populations preferred.
- Previous marketing and outreach experience preferred.
- May consider other relevant combinations of training and experience.
- Valid Wisconsin driver's license and access to personal transportation, or other reliable means for city and rural travel in all types of weather.
- Demonstrated success in leading others to accomplish health and/or community goals.
