Home Health RN Responsibilities & Duties:
- Coordinate with referring physicians to establish the initial plan of care after performing initial patient assessment, and follow/implement subsequent physician’s orders
- Evaluate and treat patients using the most current technology and practices
- Participate in clinical outcomes monitoring, follow up, and agency performance improvement initiatives
- Supervise and educate LNVs and Home Health Aides performance in following established POC
- Ability to work in a collaborative environment with an interdisciplinary team
- Travel the San Antonio and surrounding areas to include Boerne, Castroville, New Braunfels
If you meet these qualifications:
- Texas RN License
- One Year of Home Health Experience preferred
- Valid Driver’s License & Current Auto Insurance
