Position Summary:

Coordination of the plan of care, provide RN initial and ongoing reassessments, share on-call, travel to client homes within FSHC service area, and instruct/supervise the caregiver staff in the delivery of care to clients.

Essential Duties & Responsibilities:

Practices within the scope of license, certification, or registration, where applicable, and as required by law and regulation
  • Perform initial assessment at client’s home
  • Develop and implement the plan of care in collaboration with client/client’s representative
  • Update assessments and Plan of Care a minimum of every 90 days via in person (or telehealth when permitted) client visits
  • Document assessments and services in EMR and paper
  • Teach, assign, delegate, supervise caregivers to implement the plan of care
  • Educate the client/client’s representative about safe provision of care
  • Practice Environmental safety and Infection Control Principles
  • Share 24 hour on-call with other staff RNs

Other Duties:

  • Work to achieve performance goals, pursue progress toward individual development plan (IDP) and demonstrate Four Seasons values and competencies
  • Participate in department and staff meeting as required
  • Participate in ongoing education and training offered by the agency and/or required by NCBON or as part of individual education plan
  • Comply with company policies, procedures, guidelines, and Mission Statement
  • Practice safety and infection control methods
  • Perform other, but similar, duties as assigned

Knowledge, Skills & Abilities:

  • Knowledge of Federal, State and Local regulations, and Accreditation requirements as applicable
  • Knowledge of HIPAA privacy rules and regulations
  • Knowledge of electronic medical records systems
  • Knowledge of nursing services, personal care and safety, infection control, and home management skills
  • Knowledge of the English language
  • Demonstrate basic data entry skills
  • Demonstrate excellent organization skills
  • Demonstrate excellent interpersonal skills
  • Demonstrate excellent written and oral communication skills
  • Ability to work under pressure to meet deadlines
  • Ability to take initiative and utilize innovative techniques and ingenuity
  • Ability to interact with various departments throughout the organization
  • Ability to work flexible hours
  • Ability to participate as a team player to coordinate projects
  • Ability to plan, prioritize and coordinate multiple projects and client cases
  • Demonstrate ability to effectively collaborate and facilitate the work of a group
  • Demonstrate understanding of administrative support services, private and 3rd party payor options
Minimum Qualifications:

Experience:
  • Two years of clinical experience preferred
Licensures & Certifications:
  • Valid NC RN license
  • Valid Driver’s License and proof of automobile insurance
  • CPR
Summary of Physical Requirements:

  • Walking, standing, sitting, bending, reaching, twisting, crouching/squatting, kneeling
  • Flexion/Extension of arms/wrist
  • Hand dexterity to include gripping, pushing/pulling and writing
  • Lifting/carrying of 20-50 lbs.
