WHAT YOU CAN EXPECT
Assists patients, families, clinical staff and providers via telephone with clinical issues, problems and questions.
Reports change in patient status to appropriate clinician.
Participates in case conferences and interdisciplinary team meetings and coordinates changes in physician orders with clinical staff and providers.
May provide nursing visits as required to deliver patient care according to home health policies.
Completes required documentation within the established timeframe.
Receives physician orders, providing follow-up with appropriate clinical staff; may include scheduling.
QUALIFICATIONS
Associate's Degree- Required
2 years Long Term Care- Required
1 year Home Health- Preferred
Cardiopulmonary Resuscitation (CPR) - Required
Registered Nurse (RN) - Required
Auto Insurance - Required
Drivers License - Required
