What are the responsibilities and job description for the CMA/Options/HDM Assistant position at Fusion Medical Staffing?
Assists Options Counselors in the provision of the services necessary to implement the care plan as directed by the Options Counselor.
ESSENTIAL DUTIES AND RESPONSIBILITIES include the following. Other duties may be assigned. Obtains necessary medical and assessment information from medical personnel (i.e., physician, facilities) for initial assessments. Follows up on RFAs and acquire appropriate scripts and bids for home modifications and equipment needs. Assists clients with applications for benefits not typically discussed with Option’s Counselor e.g., HDM, MA, EAP, Housing, etc. Assists Options Counselor with follow-up requests and duties as assigned and/or requested. Attends workshops, meetings, and trainings as directed by supervisor. Performs clerical duties such as filing, printing, scanning and mailing. Conducts client visits to obtain signatures as needed and/or requested. Mails appropriates information/resources to clients. Supports appropriate tracking of departmental training activities and CEUs. Updates database to ensure accurate client information. Pulls reports to assist Options Counselors and Supervisor with tracking. Completes Quality Assurance calls to clients/care givers. Assists with community transition fund shopping as needed.

EDUCATION and/or EXPERIENCE Associate's degree (A. A.) in Human Services or equivalent from two-year college or technical school preferred. If not degreed, six months to one year related experience and/or training; or equivalent combination of education and experience.
