The Bridge to Home Housing Navigator and Supports Worker is responsible for advancing the mission of GHF by client- directed housing navigation, housing stability case management and other support services intended to help clients secure and maintain affordable housing.
Qualifications
· Bachelor’s degree in social work, psychology, or other human services field
· At least 1 year of experience in the social service field
· Experience serving persons with mental illness, chemical dependency, cognitive impairments, or dual diagnosis and those who are/were unhoused
· Excellent verbal and written communication skills
· Ability to document interactions with client
· Able to set priorities, work both independently and as a member of a team
· Current driver’s license and auto insurance
· SOAR certified or willingness to secure SOAR certification upon hire
Job Duties
· Engage and build relationships with clients to enhance interest and engagement in available and desired support services
· Assist with gathering necessary program eligibility and enrollment documentation for client files and otherwise maintaining complete client files
· Assist with identifying skills, supports, and resources needed to ensure housing success while increasing independence and wellness
· Complete goal plans with each client that include short and long-term goals, especially focusing on income, employment, and housing goals
· Meet with clients routinely to work on and advance household goals
· Assist tenants with obtaining and maintaining income and entitlements, including disability income, health insurance, and food stamps
· Maintain relationships with service providers and other community partners to facilitate linking clients with needed and desired resources
· Identify and build relationships with landlords to identify potential housing options
· Assist with locating emergency shelter, short-term and long-term affordable housing options based on tenants needs and desires
· Assist tenants with obtaining documents needed for leasing and certifications, including needed household personal identification (birth certificates, social security cards, state IDs)
· Coordinate the leasing and move in process with tenant, landlord, subsidy provider, utility providers, and third-party service provider
· Assist clients in securing needed furnishings and other household start-up items
· Assist with addressing tenancy issues and concerns in conjunction with tenant’s third party service providers, as necessary.
· Assist with developing strategies to preserve housing, involving tenant’s third party service provider(s) and other supports, as necessary
· Assist with obtaining permanent rental subsidies and completing documentation for subsidy providers, such as subsidized housing, community partner housing options, mainstream vouchers, or public housing
· Participate in Continuum of Care meetings and work groups, including WHAMM meetings and Coordinated Entry / Surge Campaign client case conferences
· Maintain client files, track and report on defined tenant outcomes utilizing HMIS and other GHF approved applications/forms
· Complete Client Service Notes for each service provided to tenants
· Utilize CSH best practices for Supported Housing, Housing First, and Harm Reduction
Job Type: Full-time
Pay: $35,000.00 - $43,000.00 per year
