Join Horizon as a Home Health Aide in Hospice
At Horizon, we are seeking compassionate Home Health Aides to provide direct assistance and personalized care to patients in hospice settings. Under the supervision of a Registered Nurse or Therapist, you will be responsible for assisting with personal care activities, maintaining patient health, and ensuring a supportive environment for those in need.
Key Responsibilities
Review and execute patient care plans in a home setting, promptly notifying the Case Manager of any concerns or necessary revisions.
Assist patients with daily living activities such as bathing, dressing, transfers, range of motion exercises, and toileting, while keeping them informed and involved in their care.
Complete assigned tasks in accordance with the care plan upon arrival and departure from patients' homes. Maintain accurate and timely mileage logs.
Collaborate with patients, caregivers, and Agency staff to address patient needs and communicate vital information to the Case Manager efficiently.
Perform home maintenance tasks as outlined in the care plan, including house cleaning, laundry, meal preparation, and transportation support.

- High School Diploma or equivalent
- Wisconsin CNA certification for work in Federally Certified Home Health Agencies and Hospices
- Previous experience in public-facing roles
- Current/Valid WI Driver's License and access to a reliable, insured vehicle
