Job Summary
The Social Work Fellow provides direct patient care in a variety of settings while attaining required hours for supervision to attain the Licensed Clinical Social Worker credential. Responsibilities include patient care activities within the scope of their clinical privileges commensurate with the accreditation of a master level social worker. In addition to patient care, attendance at clinical rounds and seminars, participate in supervision and education requirements for the program, timely completion of medical records, and other responsibilities will be completed as assigned.
Job Duties
- Initial and ongoing assessment of patient's psychosocial status.
- Develop assessment and treatment plan.
- Lead clinical patient care in individual and group settings.
- Record progress notes.Arrange for discharge and after care.
- Write and dictate admission notes, progress notes, procedure notes, and discharge summaries.
- Using evidence-based treatment protocols to provide care in a variety of settings.
- Provide patient education and counseling covering health status, test results, disease processes, and discharge planning.

Master's Degree-Social Work (Required)
