Social Services Assistant's primary responsibility is to support social service staff with the goal of optimizing professional care services provided to our patients/residents. The Social Service Assistant performs administrative support activities from patient admission through discharge.
Position Highlights
- Report to the Director of Social Services
- Collaborate with the interprofessional team to promote and protect patient/residents' rights and the psychosocial well-being of them. Prevents, intervenes, and reports resident abuse as mandated by law.
- Works with patients/residents, families, and significant others to provide support and resource information to promote resident/patient self-advocacy.
- Respond to issues identified by patients/residents and families to determine satisfaction with services. This includes participation in the grievance/concern Process and Civil Rights compliance i.e. securing interpreter/translator services, adaptive equipment, etc.
- Facilitate patient/resident transfer throughout the nursing center to ensure a seamless transition and patient/resident adjustment and documents accordingly.
- Participate with the health care decision-making process within the nursing center under direction of a qualified social worker and/or qualified consultant.
