Overview
We are looking for a Remote - Full Time - Social Worker (MSW) AIM (Advanced Illness Management) Program to join our team. You will report directly to the Director of Care Management AIM. You will work with patients, their families, PCP, AIM, and other health care professionals to recognize and understand the social/mental stress and/or disorder that magnifies the symptoms related to the serious illness and support/educate patients/families in understanding and executing advance care plan documents that are consistent with their wishes. This role will develop and implement plans of care, assess crisis situations to appropriately provide needed resources, and /or referrals for the patient, and to provide family and individual interventions as indicated.
About You
- Masters of Social Work from an accredited institution
- Able to attain multistate licensing as a LMSW or LCSW - licensure required
- 2 years of prior social work in healthcare setting; preferably palliative care, advanced illness management, or hospice
- Understands palliative care, hospice philosophy and issues of death/dying
- Knowledge of the impact of terminal diagnosis and anticipatory grief on psychosocial functioning, family
- dynamics, and community relations, and appropriate therapeutic interventions
- Ability to establish and maintain therapeutic relationship with patients, families, and colleagues
