Your Role
In this leadership role, you will oversee and evaluate activities related to health maintenance and promotion programs. You will manage various clinical programs, ensuring compliance with contractual obligations while achieving clinical and quality goals. Your role will involve developing essential skills and processes to effectively lead the Clinical Operations team, implement clinical programs, and manage vendor relationships.
You will collaborate with the Market Operations team, report to the Director of Nursing and Clinical Operations, and lead a team that includes Nurse Care Managers, Social Workers, Care Coordinators, and Transition of Care Coordinators. Partnering with physician leaders will be crucial for successful team and market operations. This remote position requires proximity to the Albany market for in-person meetings and visits as necessary.

The base pay range for this position is $94,300 - $108,400, determined by experience, education, skills, and organizational needs. In addition to base pay, we offer a comprehensive rewards program, including:

- Provide oversight of interdisciplinary teams, including Nurse Care Managers and Social Workers.
- Monitor clinical metrics affecting patient outcomes and team performance.
- Understand HEDIS measures to identify and address gaps.
- Manage patient complaints and grievances, ensuring timely resolution.
- Oversee the referral processes for healthcare services.
- Foster effective team dynamics to meet key performance indicators.
- Manage direct admissions to skilled nursing facilities by ensuring appropriate documentation.
- Participate in the hiring process and collaborate with recruiters.
- Conduct performance reviews and mentor team members for professional growth.
- Collaborate to develop innovative solutions to healthcare barriers.
- Work with market leaders to educate and onboard team members.

- Reliable access to high-speed internet for remote communication.
- Degree from an accredited nursing program; BSN preferred.
- Active Registered Nurse license.
- Minimum of 2 years of hospital clinical experience within the last 10 years.
- Minimum of 2 years of ambulatory clinical experience, preferably in care management.
- Experience with data-driven strategy execution.
- Proven track record of executing clinical programs successfully.
- Proficiency in Microsoft Office suite.
- Experience managing interdisciplinary teams.
- Preferred experience in home health care, health plans, or hospice.
- Knowledge in Performance Improvement is an advantage.
- Availability to travel to various work sites as needed.
The base pay range for this position is $94,300 - $108,400, determined by experience, education, skills, and organizational needs. In addition to base pay, we offer a comprehensive rewards program, including:
