LPN Home Health Care

Giving Home Health Care is a patient centered company delivering in home services to aid our patients in being able to live safely in the comfort of their homes.

We are currently seeking RNs/LPNs to work with patients in Carlsbad, NM area.

Availability ranging from once a month visits, to a few times a week.

Duties Include

Complete nursing assessment

• Complete full body assessment with special focus on the patients covered diagnosis
• Monitoring for changes in condition such as weight loss/gain, self-care abilities, indicators of disease progression
• Home assessments for DME needs
• Pulmonary assessments
• Take Vitals
• Assess for indicators of infections
• Medication reconciliation & monitoring
• Provide education and training to patient, family and home health aide on disease process.
• Communicate with case manager and outside providers PRN to ensure there is a continuity of care
• Electronic charting
• Assisting a family in being able to keep their loved one in the home
• Other duties based on patient care plan

Rate of pay is up to $32.00 -$45.00 per hour

Job Type: Part-time

Pay: $33.00 - $45.00 per hour

Requirements : Current nursing License, must have a dependable and reliable transportation
