Job Description: We are seeking a compassionate and experienced Nurse Practitioner with a specialization in geriatrics to join our dedicated team at our assisted living facility in Sheridan, WY. The ideal candidate will have a strong commitment to providing high-quality care to our elderly residents, ensuring their health, comfort, and well-being.
Key Responsibilities:
- Provide comprehensive healthcare services to elderly residents.
- Conduct regular health assessments and develop individualized care plans.
- Manage chronic illnesses and provide acute care as needed.
- Collaborate with physicians, nurses, and other healthcare professionals to coordinate care.
- Educate residents and their families on health management and disease prevention.
- Monitor and document residents’ medical progress and treatment plans.
Qualifications:
- Current Nurse Practitioner license in the state of Wyoming.
- Certification in Geriatrics or substantial experience working with elderly patients.
- Strong clinical skills and the ability to work independently.
- Excellent communication and interpersonal skills.
- Compassionate, patient-centered approach to healthcare.
- Experience in an assisted living or similar environment is preferred.
