In this role, you'll be responsible for performing OASIS assessments, developing care plans, and coordinating services to address complex chronic illness, disability, or developmental disability. You'll also oversee the provision of home nursing care, therapy, and supportive services, following their established protocols. Your nursing skills, along with the application of the nursing process, will ensure high-quality care for their patients. You will have wound care experience, as the areas we cover have high frequency wound care needs. Knowledge and experience in wound assessment, debridement, dressing selection, infection control, & wound-vac techniques is important as well as knowledge of wound healing stages & the importance of proper wound care techniques.

Requirements:
Minimum 1 year of experience with EMR systems (Preferred)
Experience with OASIS assessments (Required)
Minimum 1 year of experience in home health as an RN (Required)
Active & clean, no marks, RN License (Required)
Wound care experience (highly preferred but not required-will definitely prioritize someone with this experience)
