Responsibilities
- Provide direct patient care, including assessments, wound care, medication administration, and patient education.
- Oversee and mentor a team of home health nurses, ensuring they provide competent and compassionate care.
- Conduct clinical assessments and develop individualized care plans for patients.
- Collaborate with physicians, therapists, social workers, and other healthcare professionals to coordinate patient care.
- Ensure compliance with all regulatory guidelines and agency policies.
- Conduct performance evaluations for staff nurses.
- Identify and implement best practices for patient care delivery within the home setting.
- Active Washington State RN license.
- Minimum 2 years of experience in home health nursing.
- Strong leadership and supervisory skills.
- Excellent communication, collaboration, and interpersonal skills.
- Ability to prioritize, delegate tasks, and manage time effectively.
- Proficient in documentation and medical record keeping.
- Valid driver's license and reliable transportation. Company care provided when you are required to travel.
