Position Summary: The Grief Support Counselor is responsible for the delivery of bereavement services to family members/caregivers for 13 months following the death of a hospice patient. These services are provided via the following methods (as deemed appropriate): phone calls, in-person sessions, virtual sessions and mailings. The Grief Support Counselor will seek and collaborate with other members of the interdisciplinary group to provide the necessary support to patients and families.
Job Qualifications:
- Bachelor’s degree in social work, psychology, counseling, pastoral ministry/chaplaincy, or related field. Master’s Degree preferred.
- Experience working in hospice/end of life care.
- Demonstrated counseling skills with individuals, families, and groups.
- Comfortable with technology and social media.
- Ability to work independently, take initiative and think creatively.
- Ability to work well with an interdisciplinary team.
- Excellent written and verbal interpersonal communication skills.
- Current Minnesota driver’s license, insurance, and reliable daily transportation.
