Position Summary:  The Medical Social Worker works as a member of the interdisciplinary team working with terminally ill patients and their families. The social worker’s services are provided in accordance with the plan of care, and under the direction of a physician.

Job Qualifications:                                         

• Masters in Social Work preferred,  from an accredited program and licensure in the state of Minnesota.
• Clinical social work experience with individuals and families
• Knowledge and experience with family systems theory, individual and family counseling, and group process
• Ability to work independently, take initiative and think creatively
• Knowledge of community resources including Medicare/Medicaid, including the hospice benefit, county, state and private social service agencies
• Critical thinker  
• Ability to work well with an interdisciplinary team
• Excellent written and verbal interpersonal communication skills
• Current Minnesota driver’s license, insurance and reliable daily transportation
