What are the responsibilities and job description for the Per Diem RN - Non Medical Home Care position at Griswold Home Care Company Office?
Our Per Diem RN uses their clinical expertise reviewing client cases to contribute to our purpose of giving people the help they need to live in the place they love. This team member will also be directly responsible for clinical skills and education of our caregivers. This position is focused on our Bulter, NJ territory. Experience working with Seniors and Non-Medical Home Care is a plus.
Compensation:
$35 to $40 per hour
Specific Responsibilities:
- Prepare and sign the client care plan that specifies the services requested by the client.
- No less than once during each 30-day period during which the CHHA is rendering services in the home care setting:
- I. Inquire of the CHHA and such other persons as may be necessary whether the care plan is adequate to meet the client’s needs; and 2. Make reasonable inquiry to determine whether the care plan is being followed appropriately by the CHHA.
- Review the case notes of the clients every 30 days.
- Ensure that the CHHA demonstrates he or she is competent to perform the client care, including review of care logs & health records collected by the office staff for accuracy.
- Responsible for client care monitoring every 60 days and communication with other appropriate team members and supervisors.
- This includes a visit to the client’s home to assess any change in the client’s requested services, a revision of the care plan to reflect any change, a documented review of the updated care plan with the CHHA, and the evaluation of the satisfaction of care by the client and/or family of client.
- Follows agency and CHAP policy regarding procedures or duties that a CHHA/RN may perform.
- Complete the CHHA Competency and Skills checklist at orientation.
Skills and Qualifications:
- Experience in working with seniors and non-medical home care is a plus.
- The RN must be a registered nurse (RN) in good standing holding a BS degree in Nursing OR a registered nurse (RN) in good standing having three (2) years combined public health nursing and progressive professional responsibilities in public health nursing.
- The RN Supervisor shall maintain all necessary information for licensure and renewal, which information shall be kept on file.
- Must successfully pass a criminal background and reference check as required by state regulations.
- The RN Supervisor should be a compassionate, caring and competent individual. The mission of the company should coincide with his/her approach to care for agency’s clientele.
- Must receive appropriate health clearances, including immunizations for tuberculosis.
