Position Title: Job Coach

Program: Supported Employment

Reports To: Director of Employment Services

$2,000 Sign on Bonus

Schedule: Full time, Flexible Schedule including evenings and weekends

Salary: $21 / hour

Job Overview:

Participate as a member of the interdisciplinary team, representing the interest, and needs of the individuals served in securing community-based employment by providing job coaching, job skills training, job interest exploration, and teaching job seeking skills.

Provide assistance to adults with Intellectual/Developmental Disabilities in the implementation of goals/objectives, daily programs, ADLs, and community participation. Ensure programmatic planning and implementation of services as it relates to Department of Developmental Services (DDS) regulations.

Oversee assigned caseload and collaborate with outside agencies, residential programs, families, and other service providers.

Essential Functions of Position:

  • Assist with job coaching out at community worksites, both during Grow business hours and outside of traditional business hours (nights/weekends), and transport individuals to and from worksites and their residences as needed to ensure smooth daily operations.
  • Assist with small group implementation during programming on work-related skills, both in planning, grouping, and service delivery. This includes developing and providing in-house and community-based work preparedness training for individuals.
  • Maintain good relationships with outside employers while also assisting the Supported Employment team to locate new employment opportunities and leads for job placements for individuals served.
  • Assist individuals in maintaining good quality job performance consistent with employer expectations and to resolve conflicts effectively and efficiently.
  • Provide initial and ongoing training to people served at community employment sites to ensure that they acquire needed skills to successfully maintain their employment.
  • Participate in the ongoing assessment, modification, and development of career goals for individuals served.
  • Assist individuals and families to understand and coordinate benefit planning related to SSI/SSDI impact to employment and assist with developing plans to remove barriers to work.
  • Tracks client utilization of services, maintains program statistics, and completes required documentation for billing in a timely manner.
  • Develop training strategies for individuals in their respective jobs.
  • Assist the Supported Employment team with all necessary paperwork, documentation, billing, etc.
  • Transport individuals to and from home, worksites, community outings, etc. using work-provided vehicles.
  • Attend and participate in scheduled meetings as required, both for staff and individuals.
  • Act as an advocate for the individuals served.
  • Assists and attends to the personal care needs of people served as needed.
  • Perform duties consistent with agency/program policies, procedures and mission statements.
  • Maintain current First Aid, CPR, and other required training.

EDUCATION and/or EXPERIENCE

High School Diploma or GED. At least two (2) years of experience working with individuals with Intellectual/Developmental Disabilities. Associate’s/bachelor’s degree in human services or related field preferred. Demonstrated ability to work both independently and collaboratively with a culturally diverse population of individuals, managers, customers and the general public. Perform other related work duties as assigned by the supervisor.
