Position Summary: The Medical Concierge identifies, develops, and maintains relationships between the agency and hospitals, nursing homes, physicians and community organizations that result in the acquisition of patient referrals for the agency.
Essential Job Expectations and Procedural Responsibilities:
1. Coordinate services for patients discharged from hospitals and other referral sources into the home health program after patient/referral source has identified Guardian Angel as the home care provider.
2. Obtain valid Medicare and other referrals as outlined in the offer letter or established by Marketing management.
3. Identify patient’s Primary Care Physician and obtain authorization for home health services from
Primary Care Physicians who will sign orders.
4. Discuss any information or requests for special attention with the Clinical Supervisor before contacting field staff.
5. Obtaining doctors signatures for various home care orders and documents within time limits established by payer (typically 30 days).
6. Assists in the development of the patient's plan of care after discharge from the hospital.
7. Coordinate home care needs with the agency in accordance with the physician's plan of care.
8. Assists in the resolution of issues between families, referral sources, patients and the agency.
9. Participates in case conferences with supervisory and field personnel.
10. Maintains forms, reports, and statistical data including input into marketing tracking application.
11. Identifies opportunities for organizational improvement and participates in quality performance improvement activities.
12. Attends meetings, participating in both external and internal committees and organizations.
13. Participates in expos, health fairs, and other marketing venues when requested.
14. Submits timely and accurate expense reports within budget allotted.
15. Represents the agency at all times in a professional, ethical and loyal manner.
16. Complies with agency policies and procedures.
17. Any other duties assigned by supervisor.
18. Pre-plan daily & weekly sales activities.
19. Performs other duties as required
Education/Qualifications: Bachelor's degree in business highly desirable. Graduate of an accredited school of professional nursing. BSN preferred. Two to five years Home Health experience or other healthcare public relations preferred. Complies with accepted professional standards and principles. Must be licensed driver with an automobile that is insured in accordance with State requirements.
Competency/Skill Requirements: Must possess excellent interpersonal and telephone skills.
Must possess commitment to excellent customer service. Knowledge of Medicare-certified Home Health’s
Care regulations. Must be able to complete multi-tasks and meet multi-deadlines.

Physical Requirements: Limited lifting in the office of files not expected to exceed 10 pounds. Driving a motor vehicle, standing, sitting, walking, bending, reaching, and stretching. Visual/hearing ability sufficient to comprehend written/verbal communication.
Environmental Conditions: Works inside and is not exposed to temperature extremes or wet conditions.
Some exposure to noise and dust is to be expected.
