The Case Manager works closely with the client and treatment team to ensure comprehensive positive treatment outcomes. Your role will include meeting with clients and their families, connecting clients with professional services and maintaining consistent communication with clients and the treatment team to assess their wellbeing and make changes to their treatment plan as needed.

Requirements:
To be considered for the Case Manager position, you will need:
HS Diploma or equivalent, Bachelors degree preferred
Experience with EMR/EHR and excellent documentation skills
Excellent communication and interpersonal skills
Experience in substance abuse treatment preferred
Case Manager Responsibilities
Collaborate with continuing care providers to connect client to appropriate after care setting, including comprehensive discharge planning
Contact appropriate referents and family members for which agency has release of information (Informing these parties of clients intent and soliciting their assistance either by phone or in person)
Facilitate interventions with clients displaying behavioral or mental instability and clients at risk for leaving Against Medical Advice (AMA)
Monitor client progress and participation with program schedule, reports problems and informs supervisor, collaborates with Clinical Supervisor/Clinical Director for additional suggestions or recommendations to assist in treatment plan success
Resolve conflicts through general and supportive advice.
Provide case management services
Maintain the confidentiality of all client and facility activities
Comply with Policies and Procedures
Accept and utilize supervision
Address problems noted by supervisor
Demonstrate willingness to accept responsibility
Complete all tasks with concern for work quality
Perform other assignments as requested by supervisor
Pay: $55,000-$75,000
