THIS POSITION REQUIRES APPLICANTS TO BE TOBACCO AND NICOTINE FREE.
The Hamilton County Health Department is a tobacco and nicotine free campus. The Hamilton County Health Department prohibits the hiring of individuals who use tobacco or nicotine products in any form. All individuals who are offered a position with the Health Department are screened for tobacco or nicotine use as part of the post-offer health screening. Individuals whose post-offer health screening results are verified positive for tobacco or nicotine use will be disqualified from employment, their job offers will be withdrawn, and they will be disqualified from employment with the Hamilton County Health Department for one (1) year from the date of the post-offer health screening.
When this position is located directly within the Federal Homeless Health Clinic, applicants for hire must meet the Federal COVID-19 Vaccine Mandate requirements and be fully vaccinated or be deemed eligible for a medical or a religious exemption. Additionally, there are a number of other positions within the Hamilton County Health Department that provide operational services/support to the Federal Homeless Health Clinic. The Hamilton County Health Department has a responsibility to respond quickly and efficiently when public health emergencies occur. All Hamilton County Health Department employees are responsible for ensuring their contact information is current and they are accessible should a public health emergency arise. The Hamilton County Health Department maintains a call down system to store contact information and mobilize staff in the event of an emergency. During a public health emergency, all staff are required to respond should a call to mobilize be made. During a public health emergency, work hours for any and all Hamilton County Health Department employees are subject to modification to meet the needs of the community.
In addition to general definition, provides care coordination to a caseload of medium to high risk families that have been referred to the CHANT model for services.
ESSENTIAL FUNCTIONS: (Essential functions, as defined under the Americans with Disabilities Act, may include the following tasks, knowledge, skills and other characteristics. In addition to those tasks found under CSS/CYSHCN the Care Coordinator will work closely with the CHANT Health Case Manager and Nurse Specialist staff to ensure that the needs of the CSS/CYSHCN clients are being met. Data and documentation will be entered into the REDCap and PTMBIS systems, will assist CYSHCN clients to obtain referrals for specialized medical services, will complete all forms based on doctor's orders. The Care Coordinator will document all contacts with or on behalf of clients/patients on CHANT/CYSHCN in REDCap and PTBMIS (computerized state network) systems.
The Care Coordinator will have knowledge and be able to enter client encounters in the REDCap system and PTBMIS system on a regular basis per program guidelines. The Care Coordinator will have knowledge and be able to complete monthly special food order request for eligible clients and make appropriate referrals to partner agencies including for home visiting services. The Care Coordinator will request medical records, mail letters to clients, and comply with the program scope of services and guidelines. A valid drivers’ license is required.

Skill establishing and maintaining effective working relationships.
Skill effectively communicating in both oral and written form.
Bachelor’s Degree in Social Services, Human Services, Behavioral Sciences, Nursing, or Health Sciences with at least one (1) year of work experience in assignment area field; A valid driver’s license is required.
PHYSICAL REQUIREMENTS:
Work requires working with high risk, confrontational, mentally and physically ill, and economically disadvantaged populations with exposure to infectious diseases. Work may involve working after regular business hours in high crime areas. Extensive home visiting is required.
